Print out the PDF file “Change of Name / Address” and “Notice of Recess
/ Withdrawal” downloaded from the Home Page. Then fill in the required
sections of the print out and send it to either of the following
addresses.
If they arrive by the 20th of each month, the changes will take place in the next month.
